In Vivo CAR-T therapy is emerging as a transformative approach to next-generation cell therapy by enabling the direct genetic engineering of T cells within the patient's body, eliminating the need for conventional ex vivo cell manufacturing. This article reviews the fundamental principles of In Vivo CAR-T, compares its advantages with traditional CAR-T therapies, and highlights recent advances in targeted gene delivery technologies, particularly viral vectors and targeted lipid nanoparticles (tLNPs). It also discusses current clinical progress, expanding applications beyond oncology, and the key scientific challenges that remain.
