Background

LAG3 (Lymphocyte-activation gene 3, CD223) is a key inhibitory receptor that is highly enriched on exhausted T cells within the tumor microenvironment, where it acts as a primary driver of T-cell exhaustion and a barrier to robust antitumor immunity. It functions by binding to ligands such as MHC class II, thereby suppressing T cell activation and effector functions. LAG3 frequently cooperates with PD-1, and their co-expression synergistically drives a deeper state of T-cell exhaustion and immune escape. Consequently, LAG3 has emerged as a validated clinical target; its blockade, especially in combination with PD-1 inhibition, has been shown to reinvigorate antitumor immunity and is now an approved therapeutic strategy in advanced melanoma.
